BASIC FUNCTION This position is responsible for performing activities associated with administration and management of pharmacy programs including but not limited to: assisting in the development, implementation and maintenance of a disease state formulary system; utilization management programs; specialty pharmacy management; medication therapy management, client relationships and support; as well as the development of innovative clinical programs. The Clinical Pharmacist will play a key role in executing our strategic priorities focused on unlocking value and quality for physicians by reducing complexity of care and elevating clinical results. NOTE: This role is hybrid/flex requiring in-office visibility three days per week, working from home the other two days. Relocation is not available; sponsorship will not be extended. /lppJOB REQUIREMENTS
- Bachelor’s Degree in Pharmacy.
- Must be a Registered Pharmacist in the United States (good standing).
- 3+ year’s professional experience in managed care and/or ambulatory care.
- Strong verbal and written communication skills.
- Knowledge of practice guidelines or practice parameters; pharmaco-economics; quality improvement processes and techniques.
- Drug information skills.
- Knowledge of medication use evaluation principles.
- Skills to handle multiple priorities and manage communications professionally to achieve issue resolution with customers, employees, or management.
- Experience using Windows based word processing, spreadsheet, presentation and relational database software.
- Ability to remain at a workstation and perform computer entry for long periods of time, as required.
~ Responsibility Focus Areas ~
- Assist in the development and maintenance of pharmacy related program tools which includes disease state formulary; client specific Medication Utilization Evaluations (MUEs) and software related reporting tools; regional practice parameters for centers of excellence; and regional pharmacoeconomic centers of excellence.
- Develop and present recommendations to accounts for managing their prescription drug program based on review and analysis of utilization and performance.
- Review potential issues involving polypharmacy, drug-drug and drug-diseases interactions, duplicate therapy and controlled substance misuse in a member’s medication profile.
- Identify opportunities for optimizing generic utilization and preferred specialty pharmacy use based on review of a member’s medication profile or an account’s utilization.
- Assist in the clinical pharmacy education of the various providers around practice guidelines, disease state formularies and formulary systems, and provider group specific reporting.
- Perform comprehensive Medication Therapy Management (MTM) reviews on individual member medication profiles and develop Medication Action Plans (MAP) and recommendations that result in optimization of medication therapy and desired outcomes.
- Communicate MAP recommendations to physicians and work with physicians to appropriately revise drug therapies for individual members.
- Assist in developing Medication Utilization Evaluation (MUE) related systems that can be applied to the pharmaceutical utilization. Develop various plans to correct inappropriate utilization patterns or the maintenance of appropriate utilization patterns.
- Assist in developing pharmacoeconomic toolsets that can be used to measure the effectiveness of clinical pharmacy services.
PREFERRED JOB REQUIREMENTS
- Managed care and/or Pharmacy Benefit Management (PBM) experience.
- Medication Therapy Management (MTM) experience, or specialty pharmacy knowledge.
